Based in Colorado, Polaris Care is dedicated to empowering individuals with disabilities to lead fulfilling, independent lives within their communities. We offer a full range of residential and community-based services, tailored to the unique needs, goals, and preferences of each person we support.

Host Home (IRSS)
A nurturing family environment where trained host families provide personalized, one-on-one support — offering a warm, stable home while maintaining each person's community ties.
ResidentialIndependent Living (IRSS)
Support for individuals living on their own — building daily living skills, managing responsibilities, and engaging confidently with their community while retaining full independence.
ResidentialGroup Home (GRSS)
A supportive, shared living environment with trained staff available around the clock — where residents build relationships, take part in daily activities, and grow their independence.
ResidentialSupported Community Connections (SCC)
Small-group and individualized activities that build real relationships and skills in the community — shopping, volunteering, recreation, and more, with member-directed choice throughout.
CommunityMentorship
One-on-one mentoring relationships that help individuals build life skills, confidence, and connections — guided by goals the individual sets for themselves.
CommunityHomemaker
Hands-on help with household tasks — cleaning, meal preparation, and home management — so individuals can maintain a safe, comfortable home of their own.
In-HomeSpecialized Habilitation
Engaging social, recreational, and skill-building activities in a safe, supportive setting — a place to connect with peers, explore new interests, and stay active in the community.
